# Compare what was explained, without choosing a winner

Preparation chapter 3

Use the same headings for both conversations. Record what each clinician explained, what you understood and what remains open. A missing note or a different phrase is a question to clarify, not a score against a clinician.

## Put the explanations on equal terms

Separate the stated goal, the rationale explained, the information discussed, the options considered and the questions still open. Do this for each conversation. If a subject was not discussed, write that; if you do not remember the explanation, mark your recollection as uncertain. These are different gaps.

Before treating two phrases as a disagreement, ask what each means in the context of your proposal. Record agreement as carefully as a difference. This is a way to organize clarification, not a method for determining which clinical approach is appropriate.

## A fictional example: Morgan has two incomplete sets of notes

Morgan heard explanations in two conversations and wants to compare them. Morgan's notes contain an apparent difference in wording, but neither set is a complete clinical account. Both columns use the same headings.

| Note heading | Conversation A: Morgan's fictional notes | Conversation B: Morgan's fictional notes |
| --- | --- | --- |
| Goal | A goal was explained; Morgan wrote a short summary to check. | Morgan understood the goal to be similar; the wording needs checking. |
| Reason for the approach | An explanation was given, but Morgan did not record how one proposed step relates to the goal. | One step was explained using a different phrase; Morgan has not confirmed whether it refers to the same point. |
| Information discussed | Morgan brought the written proposal. Other record arrangements are not noted. | The records needed for this conversation were not confirmed in Morgan's notes. |
| Options and uncertainties | Morgan has not recorded which options were discussed. | Morgan has not recorded which options were discussed. |
| Clarification task | Ask the clinician to check the summary and explain the missing connection. | Ask the clinician to explain the phrase and confirm the apparent agreement or difference. |

## Turn the apparent difference into a follow\-up

Morgan's next task is not to award points for the longer explanation. It is to identify the missing connection and ask for it to be made clear. For example: 'I used different words in these notes. Could you explain what your phrase means for the goal we discussed?' The answer can then be summarized and checked.

Keep a clinician's explanation distinct from your interpretation of it. A useful note can say 'I understood this to mean...' and leave space for confirmation. A supporting person can help preserve the wording without being asked to decide between treatments.

## Close the conversation with a clear next question

Review what has been answered and what remains unresolved. Identify which clinician or practice should clarify each item and which practical arrangements still need confirmation. A question about an explanation is different from a question about appointment scope, records delivery or a written estimate.

These notes do not select a treatment, change your current instructions or transfer care. If you are already receiving treatment, use the responsible team's contact instructions for current concerns. Discuss an actual change of care directly with the practices involved.

| Closing note | What to keep distinct |
| --- | --- |
| Answered | Your checked summary of the explanation, rather than a provider rating. |
| Still open | The exact question and who should clarify it. |
| Arrangement to confirm | Appointment scope, records, fees or timing that has not been established. |
| Current\-care concern | The contact instructions from the team responsible for your treatment. |
