# Know what you have before arranging records

Preparation chapter 2

An inventory can show what is available without deciding what a clinician needs. Keep document titles, dates, origins and status separate, then ask the receiving practice which existing material it needs and how it should be provided.

## A proposal, a record and your notes have different roles

A written proposal is the document you want explained. A record supplied by a practice is an item to identify for the receiving team, not something this guide teaches you to interpret. Your own notes capture your recollection and questions. Keep those labels visible so that a remembered explanation does not become a purported clinical finding.

SMILE\-FX's published second\-opinion information mentions existing X\-rays, scans and treatment plans. That list is not a case\-specific requirement or an instruction to arrange new imaging. Ask what is needed for the appointment you are discussing.

## A fictional inventory: available is different from confirmed

Jordan has one proposal copy and a document list. The list mentions other material, but Jordan has not established what is available or what the receiving practice wants. The example uses invented labels and dates, not a patient's records. An unknown stays unknown.

| Fictional item and type | Date supplied | Origin reference | Status | Question to confirm |
| --- | --- | --- | --- | --- |
| Proposal copy \(written plan\) | Fictional 1 October 2026 | Practice A, an invented label | Available to Jordan | Does the receiving team want this version, and how should it be provided? |
| Scan entry on a document list | Not confirmed | Practice A, an invented label | Item listed; record not held | Is an existing scan needed, and who should arrange it if requested? |
| Jordan's conversation notes | Fictional 2 October 2026 | Personal notes, not a practice record | Available; recollection unconfirmed | Which points should Jordan ask the clinician to check? |

## Confirm the arrangements before sending anything

First identify what you legitimately have. Next, ask the receiving practice which existing items are needed and whether it needs to clarify the request with another practice. Then confirm the actual delivery method and any authorization arrangements. A document being available to you does not establish that a website, email address or family member is the correct route for providing it.

Ask the practice to confirm formats, any release or signatory requirements, fees and timing. This guide does not supply those rules or a transfer process. There is no records upload here, and the consultation link is not a records\-delivery channel. Do not put health records into an unconfirmed channel.

- Which existing documents or records are needed for the proposed appointment?
- What is the practice's confirmed method for receiving them?
- Who should arrange any request or authorization, and what must be confirmed first?
- Are there costs, timing limits or missing items to clarify before the visit?

## Make the inventory useful when something changes

If Jordan later learns that an item has been requested, the status can change to 'requested' with the date of that update. It should not change to 'received' until that has been confirmed. The question column can record who needs to clarify the next step, without interpreting the record or promising when it will arrive.

Keep this as a personal preparation aid. A relative's help with an inventory does not establish authority to release records. The practice must confirm the arrangements for the individual situation.
